Ассоциация концентрации 25(ОН)D с атопическим дерматитом и его тяжестью у детей в возрасте 3–6 лет: одномоментное исследование

Аннотация

Обоснование. Связь обеспеченности детей витамином D с развитием и тяжестью течения атопического дерматита (АтД) однозначно не определена.

Цель исследования — сравнить концентрацию 25(ОН)D в сыворотке крови у детей с АтД и у здоровых сверстников с анализом ассоциации 25(ОН)D с тяжестью болезни и сенсибилизацией к аллергенам.

Методы. В исследование включали детей в возрасте 3–6 лет с АтД и здоровых сверстников. Обеспеченность витамином D оценивали по концентрации 25(ОН)D в сыворотке крови, которую определяли хемилюминесцентным иммуноанализом на микрочастицах. При концентрации 25(ОН)D 20 нг/мл устанавливали дефицит витамина D, 21–29 нг/мл — недостаточность. Сенсибилизацию к ингаляционным (клещи Dermatophagoides pteronyssinus, Dermatophagoides farinae, эпителий и перхоть кошки) и пищевым (белки коровьего молока и куриного яйца) аллергенам определяли по концентрации IgE, детектируемых методом иммунофлуоресценции.

Результаты. Концентрация 25(ОН)D в сыворотке крови у детей с АтД (n = 106) была ниже, чем у здоровых сверстников (n = 40), медиана (межквартильный размах) — 26,5(19,0–35,3) и 32,4 (27,9–37,5) нг/мл соответственно (р = 0,012). При этом недостаточность/дефицит витамина D по концентрации 25(ОН)D были отмечены более чему половины (58%) больных АтД и у каждого третьего (37%) ребенка контрольной группы, отношение шансов — 3,5 (95% доверительный интервал — 1,6–7,5). Также установлено, что концентрация 25(ОН)D была ниже при среднетяжелом (в сравнении с легким) течении АтД, а также у детей, сенсибилизированных к ингаляционным и/или пищевым аллергенам.

Заключение. У детей дошкольного возраста обеспеченность витамином D ассоциирована с АтД, степенью тяжести течения болезни и наличием сенсибилизации к ингаляционным и/или пищевым аллергенам.
