Ошибки и осложнения при хирургическом лечении пациентов с переломами дистального метаэпифиза лучевой кости

Аннотация

Цель - анализ ошибок и осложнений в хирургическом лечении больных с нестабильными переломами дистального метаэпифиза лучевой кости и пути их профилактики. Метериал и методы. Хирургическое лечение выполнено 203 пациентам (62 мужчин и 141 женщины) с 204 переломами дистального метаэпифиза лучевой кости (ДМЛК): 105 больных были оперированы в сроки до месяца после травмы (I группа), 98 - через месяц и позже (II группа). Результаты лечения оценивали с помощью морфометрических и клинических показателей, предложенной авторами схемы и опросника DASH. Результаты. Через год после операции в I группе получено 88,68% положительных исходов, во II группе - 72,45%. Частота осложнений при использовании ладонного оперативного доступа в I группе составила 4,72%, во II группе - 17,35% (р=0,0001). Число операций по удалению металлофиксаторов сократилось до 6,6% в I группе и до 11,22% - во II группе (р=0,245), что в 3,3-4,75 раза меньше по сравнению с данными литературы. Заключение. Применение пластин с угловой стабильностью в хирургическом лечении больных с переломами ДМЛК позволяет восстановить рентгенометрические и функциональные показатели кисти и кистевого сустава. Значительно меньшее число осложнений в I группе (4,72%) по сравнению со II группой больных (17,35%) при использовании ладонного оперативного доступа позволяет рекомендовать раннее, до 1 месяца после травмы, хирургическое лечение при нестабильных переломах ДМЛК.

Abstract

The purpose - to analyze errors and complications in surgical treatment of patients with distal radius fractures and to determine methods of its prevention. Material and methods. 203 patients (62 male and 141 female ones) with 204 distal radius fractures were treated using plate osteosynthesis. 105 patients were operated ear-lier than 1 month after the injury (I group), 98 - in 1 month or later after the injury (II group). The results of the treatment were assessed with morphometric and clinical indi-cators, scheme offered by the authors and DASH questionnaire. Results. 88,68% of positive results in I group and 72,45% in II group were ob-served in a year after the operation. The frequency of complications on application of palmar surgical approach was 4,72% in I group and 17,35% (р=0,0001) in II group. The rate of hardware removal reduced to 6,6% in I group and to 11,22% - in II group of patients (р=0,245) that is in 3,3-4,75 times lower than the data given in the literature.
