Профилактическое лечение хронической головной боли напряжения и хронической мигрени

Аннотация

Профилактическая терапия головной боли (ГБ) направлена на снижение частоты, длительности и тяжести приступов и показана пациентам с хронической формой головной боли напряжения (ХГБН) и хронической мигренью (ХМ), а также в случаях, когда пациент сам желает получать превентивную терапию для восстановления качества жизни. Цель работы заключается в сравнительной оценке эффективности 2 различных превентивных стратегий с использованием монотерапии амитриптилином и амитриптилина в сочетании с мягкими техниками мануальной терапии. Обследовано 35 пациентов: 24 человека с ХГБН (группа 1) и 11 с ХМ (группа 2). Пациенты обеих групп методом случайной выборки были разделены на подгруппы (подгруппы 1а, 1б, 2а и 2б), пациенты подгрупп 1б и 2б в начале превентивной терапии дополнительно получали лечение с использованием мягких техник мануальной терапии. Клини- ко-неврологическое и психометрическое обследование проводилось до и после профилактического лечения. Под влиянием обеих терапевтических стратегий отмечалась достоверная позитивная динамика в виде редукции алгического синдрома, уменьшения выраженности и частоты приступов ГБ, коморбидных нарушений, нормализовалось эмоциональное состояние пациентов. Только комплексная терапия амитриптилином в сочетании с мягкими техниками мануальной терапии привела к полному нивелированию болезненности перикраниальных мышц.

Prophylactic treatment for chronic tension-type headache and chronic migraine

Abstract

Preventive therapy for headache (HA) is aimed for reduction of frequency, duration and severity of attacks and is indicated for patients with chronic tension-type headache (CTTH) and chronic migraine (CM), as well as in cases where the patient himself wants to receive preventive therapy to restore quality of life. The aim of the work was to compare the effectiveness of 2 different preventive strategies using amitriptyline monotherapy and amitriptyline in combination with soft manual therapy techniques. 35 patients were examined: 24 with CTTH (group 1) and 11 with CM (group 2). Patients of both groups were randomly divided into subgroups (subgroups 1a, 1b, 2a and 2b), patients of subgroups 1b and 2b were additionally treated with soft manual therapy techniques at the beginning of preventive therapy. Clinical neurological and psychometric examination was carried out before and after prophylactic treatment. Under the influence of both therapeutic strategies, there was a significant positive dynamics in the form of reduction of pain, decrease of severity and frequency of HA attacks and comorbid disorders, and the emotional state normalization. Only complex therapy with amitriptyline in combination with soft manual therapy techniques led to the complete elimination of pericranial muscles tenderness.
