Карциноид вилочковой железы у пациентки в возрасте 16 лет: обзор литературы и собственное клиническое наблюдение

Аннотация

Опухоли вилочковой железы представляют собой редкую патологию детского возраста. Еще реже у детей и подростков встречаются карциноиды тимуса. В обзоре литературы приведены данные по заболеваемости, морфологии, дифференциально-диагностическим признакам нейроэндокринных опухолей тимуса, клиническим проявлениям и прогностическим критериям заболевания. Подчеркиваются трудности дооперационной диагностики нейроэндокринных опухолей вилочковой железы. Для лечения карциноидов тимуса применяются хирургический, лучевой, лекарственный методы и их комбинации, что зависит от размеров опухоли, ее инвазии в окружающие органы и ткани, наличия метастазов. Приводится собственное клиническое наблюдение за пациенткой в возрасте 16 лет, которой выполнена радикальная операция по поводу карциноида вилочковой железы. Гистологическая и иммуногистохимическая картина соответствует атипическому карциноиду тимуса. Статья иллюстрирована компьютерными ангиограммами и операционными фотографиями.

Abstract

Tumors of the thymus gland are rare pathology in children. Even rarer in children and adolescents meet carcinoids of the thymus. In the literature review, data on morbidity, morphology, differential-diagnostic criteria of neuroendocrine tumors of the thymus, clinical manifestations and prognostic criteria of the disease. Underlined difficulties preoperative diagnosis of neuroendocrine tumors of the thymus. For the treatment of carcinoids of the thymus apply surgical, radiation and drug methods and their combinations, depending on the size of the tumor, its invasion into surrounding tissues and organs, the presence of metastases. Is its own clinical observation of the patient is 16 years old, which made radical surgery about carcinoid the thymus gland. Histological and immunohistochemical pattern corresponds to the atypical carcinoid thymus. The article is illustrated with computer angiograms and operating photos.
