Аннотация
В последние годы обозначился интерес врачей интенсивной медицины к проблеме послеоперационной когнитивной дисфункции. Однако вопросы диагностики, профилактики и лечения нарушений ментального статуса далеки от разрешения. Оптимизация седации в отделениях реанимации и интенсивной терапии привлекает заметное внимание современных клиницистов. При этом выбор оптимального препарата для седации является непростой задачей. В связи с этим было проведено исследование, включающее изучение распространенности и структуры делирия, а также оценку эффективности препаратов для седации у геронтологических пациентов ортопедо-травматологического профиля.

STRUKTURE AND PREVALENCE OF DELIRIUM IN GERONTOLOGICAL PATIENTS IN ORTHOPEDICS; POSSIBILITIES OF PROLONGED SEDATION WITH ALPHA2-AGONISTS
Abstract
Postoperative delirium is a frequent complication that develops in patients operated on in emergency and planned order, and also increases mortality among elderly patients with fractures of the femur. 207 gerontological patients who entered the Scientific and Research Institute of Traumatology and Orthopedics of Astana in the period from 2016 to 2018 were examined. They studied the epidemiology of delirium, evaluated the effectiveness of sedative therapy with dexmedetomidine and haloperidol according to the RASS scale. The effect of dexmedetomidine was significant and was expressed in a 53 % decrease in the duration of delirium with respect to haloperidol (p < 0.05). Dexmedetomidine provided more controlled and safe sedation compared with haloperidol.
