ВНУТРИБОЛЬНИЧНЫЙ ИНСУЛЬТ У ПАЦИЕНТОВ С СЕРДЕЧНО-СОСУДИСТЫМИ ЗАБОЛЕВАНИЯМИ: ЛИТЕРАТУРНЫЙ ОБЗОР
Аннотация
В настоящее время в зарубежной литературе уделяется большое внимание актуальной медико-социальной проблеме острых нарушений мозгового кровообращения, однако ограниченной остается информация о пациентах с внутрибольничными инсультами. При этом, по данным различных авторов (Foulkes MA, Kelley R, Blacker DJ), частота встречаемости внутрибольничного инсульта достигает 15 %. От 2,2 % до 17 % всех внутрибольничных инсультов имеют симптомы во время госпитализации у пациента, изначально поступившего с другим диагнозом или для определенной процедуры. Ежегодно только в США выявляется от 35000 до 75000 случаев внутрибольничных инсультов. Частота инсульта после шунтирующих операций на сердце составляет около 2 %, при использовании искусственного кровообращения она достигает 2,5 %, почти у четверти пациентов инсульт приводит к летальному исходу. После операций на клапанах сердца внутрибольничный инсульт развивается в 1–10 % случаев.
Были проанализированы источники из статей и журналов, посвящённых данной тематике, что привело к выводу: внутрибольничные инсульты наиболее распространены у пациентов с сердечно-сосудистой патологией. Были проанализированы основные причины и механизмы развития внутрибольничного инсульта.

IN-HOSPITAL STROKE IN PATIENTS WITH CARDIOVASCULAR DISEASES: LITERATURE REVIEW
Abstract
Currently, foreign literature pays great attention to the urgent medical and social problem of acute cerebrovascular accident, however, information on patients with in-hospital strokes remains limited. Moreover, according to various authors (Foulkes MA, Kelley R, Blacker DJ), the incidence of in-hospital stroke reaches 15 %. Between 2.2 % and 17 % of all in-hospital strokes have symptom onset during hospitalization in a patient originally admitted for another diagnosis or procedure. With an estimate of 35 000 to 75 000 in-hospital strokes in the United States each year. The frequency of in-hospital stroke after shunting heart operations is about 2 %, when using cardiopulmonary bypass, it reaches 2.5 %, in almost a quarter of patients, stroke leads to death. After operations on the heart valves, in-hospital stroke develops in 1-10% of cases.
We analyzed sources from articles and journals devoted to this topic, which led to the conclusion that in-hospital strokes are most common in patients with cardiovascular disease. We analyzed the main causes and mechanisms of the development of in-hospital stroke.
