ВК-вирус-ассоциированный интерстициальный нефрит после аллогенной трансплантации гемопоэтических стволовых клеток

Аннотация

ВК-вирус наиболее часто упоминается в контексте трансплантации почки как причина тяжелых нефритов и отторжений трансплантатов. В контексте трансплантации гемопоэтических стволовых клеток этот вирус чаще фигурирует как причина тяжелых геморрагических циститов. Роль ВК-вируса в развитии нефритов, ассоциированных с трансплантацией гемопоэтических стволовых клеток, невелика, а в педиатрической практике ограничена лишь единичными наблюдениями. В данной работе представлены два клинических случая ВК-вирус-ассоциированного интерстициального нефрита у пациентов в возрасте 5 и 23 лет, перенесших аллогенную трансплантацию гемопоэтических стволовых клеток, а также обзор литературы по данной проблеме.

Abstract

BK-virus has been identified as the main cause of polyomavirus-associated nephropathy, a major cause of renal allograft failure. Hemorrhagic cystitis is a major complication of BK-virus infection after hematopoietic stem cell transplantation. The reports of polyomavirus-associated nephropathy after hematopoietic stem cell transplantation (HSCT) are surprisingly very few. Here we describe two patients received an unrelated donor HSCT and developed a biopsy-proven BK nephropathy, which contributed to his renal failure.
