Ишемический приапизм как первая манифестация хронического миелоидного лейкоза

Один из редких этиологических факторов приапизма — гематологические заболевания, в частности хронический миелоидный лейкоз (ХМЛ). Приапизм может быть первым клиническим проявлением ХМЛ. В статье описан клинический случай ишемического (веноокклюзивного) приапизма длительностью 18 ч, возникшего на фоне не диагностированного ранее ХМЛ. В дальнейшем диагноз ХМЛ был поставлен на основании исследования периферической крови и пунктата костного мозга. Лечебная тактика включала незамедлительную пункцию кавернозных тел с аспирацией крови и внутрикавернозной инъекцией раствора фенилэфрина. После купирования приапизма проведена терапия ХМЛ, что позволило избежать рецидивов приапизма у данного пациента.

Ischemic priapism as the first manifestation of chronic myeloid leukemia

Priapism may occur in patients with hematological disorders with excessive white blood cell counts, such as chronic myeloid leukemia (CML). Priapism may be the first clinical manifestation of CML. At this work we report a case of a young adult who presented with ischemic (low-flow or veno-occlusive) priapism which had persisted for the last 18 hours as the first manifestation of CML. CML was diagnosed on the basis of peripheral blood and bone marrow examination . Treatment of the priapism was initiated by corporal aspiration and intracavernous phenylephrine irrigation. Furthermore, along with management of priapism, appropriate treatment of CML allows to reduce risk of recurrence of priapism in this patient.
